Use this form to authorize 845 Data to transfer (port) your existing telephone numbers from your current service provider to 845 Data. Accuracy matters: the account name, service address, account number and billing telephone number below must match your current provider's records exactly. Optional, but it is the single best way to avoid a rejected port. Your bill shows the exact account name, service address, account number and billing telephone number your current provider has on file, so we can match them character for character before we submit. A clear photo of a paper bill is fine, and you can attach more than one page. Up to 10 MB per file.
List every number you want moved to 845 Data, one per line, with area code. If you are porting the entire account, list the numbers anyway so we can confirm the count.

If you are porting every number, the account with your current provider will normally close once the port completes. If you are keeping some numbers with them, that account stays open. Tell us either way so we can flag it on the request.
A request, not a commitment. 845 Data confirms the actual firm order commitment (FOC) date with you once your current provider accepts the request. Timelines vary widely - some ports are done in about 3 business days, others take 2 weeks or more.
